The purpose of this email is to provide more specific SAMS Aging Waiver plan auto-closure update information to Service Coordinators (SCs) providing services to Aging Waiver participants residing in the Community HealthChoices (CHC) Southeast Zone (Phase-2) in their caseloads.

 

·         OLTL previously notified the SCEs as part of the attached email update’s CHC Phase- SAMS Instructions that a SAMS Aging Waiver auto-closure script would be run in the month of October to systematically end-date service plans and services effective 12/31/2018 for all Aging Waiver participants in the Southeast Zone who are transitioning to CHC on 01/01/2019.  Due to additional system considerations, the SAMS Aging Waiver auto-closure script was delayed until November.

 

·         This email update confirms that there will be an ongoing series of weekly SAMS Aging Waiver auto-closure scripts run for this purpose.  The first SAMS Aging Waiver auto-closure run is anticipated to take place during the weekend of 11/17/2018 to 11/18/2018.  SAMS Aging Waiver enrollments, care plans and service plans in Bucks, Chester, Delaware, Montgomery, and Philadelphia Counties will display an end date of 12/31/18. Please note that any corresponding service orders beyond 12/31/18 will be deleted as well for the impacted counties.

 

·         Due to the nature of what is required with the implementation of the first SAMS Aging Waiver auto-closure script, OLTL is requesting SCs with SAMS Aging Waiver cases in any of the five CHC Phase-2 counties (Bucks, Chester, Delaware, Montgomery and Philadelphia) to please refrain from making any SAMS Aging Waiver file data updates or data changes from 6:00PM on Friday, 11/16/2018 through 7:30 AM on Monday, 11/19.  Please do not work on any SAMS Aging Waiver files or service plans in any of the five CHC Phase-2 counties during this time period.  (SCs may still continue to conduct plan updates or data changes on SAMS Aging Waiver files in other residential counties during this time period.)  

 

·         OLTL will continue to review and approve SAMS Aging Waiver service plan requests with service plan dates that go beyond 12/31/2018 for any plan requests that are already pending before the first SAMS Aging Waiver auto-closure script runs in the five CHC Phase-2 counties.  The weekly SAMS Aging Waiver auto-closure scripts will pick up these plans.  However, OLTL is requesting SCs with any new SAMS Aging Waiver service plan requests in any of the five CHC Phase 2 counties to please start using 12/31/2018 as the service plan end-date and 12/31/2018 for service end-dates for any Aging Waiver service plan requests that need to be submitted to OLTL from 11/20/2018 through 12/31/2018.

 

Note: Aging Waiver participants residing in Zones 3 through 5 enrolled in Aging Waiver will not have SAMS enrollments, care plans or service plans automatically end-dated at this time.
