Provider Revalidation – Register New Service Location

Providers who have completed revalidation and received a brand new Provider Type 59 service location(s) will need to register the new Provider Type 59 service location(s) in PROMISe™.  In cases where a provider already had an existing Provider Type 59 service location or where service locations were consolidated into an existing Provider Type 59 service location, registration for a new service location should not be necessary.

Please access the link below and select “Register Now” under Provider Login to begin the registration process for your new service location.  Please refer to the Internet Help Manual under Quick Links for additional information.
