Provider Revalidation - Register New Service Location

Providers who have completed revalidation and received a brand new Provider Type 59 service location(s) will need to register the new Provider Type 59 service location(s) in PROMISe(tm).  In cases where a provider already had an existing Provider Type 59 service location or where service locations were consolidated into an existing Provider Type 59 service location, registration for a new service location should not be necessary.

Please access the link below and select "Register Now" under Provider Login to begin the registration process for your new service location.  Please refer to the Internet Help Manual under Quick Links for additional information.

Also, it is imperative that you notify the Office of Long-Term Living for changes that would affect your provider file, such as addresses and telephone numbers.  Mail to/pay to addresses, email addresses, and phone numbers can be updated electronically immediately through ePEAP, which can be accessed through the PROMISe provider portal. For any other provider file changes please notify the Bureau of Quality and Provider Management Enrollment and Certification Section at 1-800-932-0939 Option #1.
